2013-2015 Fellow Application

To apply, please click on (Preceptee Application) and save a copy to your computer. Complete the application and email to Arcy Dominguez at Adominguez@aagl.org. Applications must be submitted electronically.

Important Dates:

• Deadline to submit application: July 1, 2012
• Interviews with applicants: To be determined by each site. To be scheduled no later than September 30, 2012.
• Submission of Rank List: October 5, 2012
• Notification of match results: October 26, 2012
• Program Start Date: July 1, 2013

New Site Application

We are no longer accepting applications for 2012.

Applications for 2013 will be available March 2012.

• Site visits to be scheduled: February, March, April and May 2013
• Disposition of Preceptor Application determined: June 2013
• Program start date: July 1, 2013 or July 1, 2014
