Abstract
Master Class in Gynecologic Surgery
Edited by Charles E. Miller, M.D.
Managing the Separate Uterus
The simplicity of hysteroscopic septoplasty gives experienced surgeons the opportunity to remarkably improve reproductive outcomes for patients with a septate uterus. Incision of the septum equidistant between the anterior and posterior wall and to the level of the uterine fundus--with both ostia in the visual field and with care taken to avoid trauma to the myometrium of the fundus---is achieved with the use of microscissors and a 12- or 25-degree lens. This energy-free technique avoids thermal damage and lessens the risk of adhesion formation. When a broad septum limits visualization of both ostia, sequential thinning incisions are made along each side of the septum from the apex to the fundus.
