Abstract
Master Class in Gynecologic Surgery
Edited by Charles E. Miller, M.D.
Robotic Myomectomy
Robotic myomectomy is advancing, most notably with incorporation of real-time MRI imaging, which enables the surgeon to overcome haptic limitations and to remove multiple fibroids. A 4-arm approach enables the surgeon to manipulate the uterus with one of the instrument arms instead of relying on a bedside assistant having vaginal access to do this task. Similarly, the use of two right-side robotic instruments gives the surgeon flexibility in manipulating the uterus or fibroid(s) and more self-sufficiency in performing various parts of the procedure. For enucleation, a push-spread technique is preferable over significant use of cautery. Closure of the hysterotomy can now be achieved with the use of barbed suture.
