2009-2010 Fellows Application
For physicians and residents interested in undertaking a fellowship, please click on Fellows Application. Please save a copy to your computer. Complete the application and email to Arcy Dominquez. Details on qualifications, educational objectives, and learning objectives are included in the application.

The deadline for returning completed applications is July 1, 2008.

2009-2010 Preceptors Application
If you're an experienced endoscopic surgeon looking to be a preceptor in a fellowship, please click on Preceptor Application. Please save a copy to your computer. Complete the application and email to Arcy Dominquez. By becoming a preceptor, you'll be passing on your knowledge to future generations of doctors.
